An annual physical exam — sometimes called a yearly checkup, wellness visit, or preventive care visit — is one of the most valuable investments you can make in your long-term health. Yet many adults either skip it entirely or walk in unsure of what to expect. As a board-certified internist in Wildomar, CA, I want to change that by walking you through exactly what happens at a thorough annual physical at Patel Primary Care.

Why Annual Physicals Matter

The biggest value of an annual physical isn't treating illness — it's preventing it. Many serious conditions, including hypertension, high cholesterol, diabetes, and certain cancers, develop silently with no obvious symptoms. A yearly visit gives your doctor the opportunity to catch these issues early, when they're most manageable.

Beyond screening, an annual physical helps establish a baseline for your health. Over time, your doctor can track trends in your lab values, blood pressure, and weight that might not be alarming in isolation but indicate a meaningful pattern over years. It also builds the relationship between you and your physician — so that when something does go wrong, your doctor already knows your history, your medications, and your goals.

Most insurance plans, including Medicare, cover one annual preventive physical per year at no cost to you. Don't leave this benefit unused.

How to Prepare Beforehand

A little preparation goes a long way toward making your visit more productive. Here's what I recommend before your annual physical:

  • Fast if needed. If your visit includes a lipid panel or fasting glucose, you'll need to fast for 8–12 hours. Drink water and take your usual medications unless told otherwise. Call our office to confirm.
  • List all medications and supplements. Include prescription medications, over-the-counter drugs, vitamins, and herbal supplements — with dosages if possible.
  • Write down your questions and concerns. It's easy to forget things once you're in the room. Having a written list ensures nothing gets missed.
  • Gather recent records. If you've seen specialists, had imaging, or had lab work done elsewhere in the past year, bring those results or have them sent to our office beforehand.
  • Know your family history. First-degree relatives (parents, siblings, children) with heart disease, diabetes, cancer, or other conditions are important for your screening schedule.
  • New patients: arrive 15 minutes early to complete intake paperwork and ensure a full appointment time.

What Happens During the Exam

At Patel Primary Care, a complete annual physical typically follows this structure:

1. Health History Review

We start by updating your medical history — any new diagnoses, hospitalizations, surgeries, or significant health events since your last visit. We'll also review your current medications, allergies, and any concerns you've noticed in the past year.

2. Vital Signs

A medical assistant will measure:

  • Blood pressure (both arms if indicated)
  • Heart rate and rhythm
  • Respiratory rate
  • Temperature
  • Height, weight, and BMI
  • Oxygen saturation

These baseline measurements are critical reference points. A blood pressure of 138/85 might be fine in isolation, but if it was 118/76 last year, that trend matters.

3. Head-to-Toe Physical Examination

I'll perform a thorough hands-on examination that typically includes:

  • Head & neck: lymph nodes, thyroid gland, jugular veins
  • Eyes: pupil response, conjunctiva (signs of anemia)
  • Ears, nose, throat
  • Heart: rate, rhythm, murmurs
  • Lungs: breath sounds, air entry
  • Abdomen: organ size, tenderness, bowel sounds
  • Skin: moles, rashes, lesions
  • Extremities: reflexes, pulses, edema
  • Neurological: gait, coordination, cognition as appropriate

4. Preventive Screenings Discussion

Based on your age, sex, family history, and risk factors, we'll discuss which screenings are due. This might include a referral for a mammogram, colonoscopy, bone density scan, or skin cancer screening — depending on what's indicated for you specifically.

5. Lifestyle Counseling

We'll talk honestly about diet, physical activity, sleep, stress, smoking, and alcohol. These conversations matter — lifestyle factors are the single biggest driver of chronic disease risk. I don't lecture; I collaborate. We'll set realistic goals together.

6. Immunization Review

We'll check whether you're due for any vaccines — flu, pneumonia, shingles (Shingrix), Tdap, COVID-19 booster, or others based on your age and history. We don't give vaccines in our office, so we'll tell you where to get any you need.

Lab Work & Screenings

Most annual physicals include a lab order. Common tests ordered at Patel Primary Care include:

  • Complete Blood Count (CBC) — checks for anemia, infection, and blood disorders
  • Comprehensive Metabolic Panel (CMP) — kidney function, liver function, electrolytes, blood sugar
  • Lipid Panel (fasting) — total cholesterol, LDL, HDL, triglycerides
  • HbA1c — average blood sugar over 3 months, screens for diabetes and prediabetes
  • Thyroid Function (TSH) — especially for women over 35 and anyone with symptoms
  • Urinalysis — screens for kidney disease, infection, diabetes
  • Additional tests as clinically indicated (PSA, STI screening, vitamin D, iron studies, etc.)

Lab results are typically available within a few days and will be reviewed with you — either at a follow-up visit or via the Patient Portal with a message explaining what they mean for your health.

Don't just accept "your labs are normal" without context. Ask your doctor what your numbers actually are and how they compare to last year. Trends matter as much as single values.

After Your Visit

Before you leave, you'll receive a clear summary of:

  • Any new diagnoses or findings from today's visit
  • Medications that were added, changed, or discontinued
  • Referrals or follow-up tests ordered
  • Your personalized health goals for the coming year
  • When to schedule your next visit

You can also access your visit summary, lab results, and messages from Dr. Patel through our secure Patient Portal at any time.

If you haven't had an annual physical this year — or if it's been more than a year since your last checkup — I encourage you to schedule one. It's one of the most proactive things you can do for your health.
